We offer limited appointments at our Franklin location. Call (615) 936-0249 to ask about availability.
Preparing for your visit
Include your child's IEP (individualized Education Plan) or IFSP (Individual Family Service Plan) in the pre-appointment packet you mail to us.
Appointments with your medical providers last about one hour. Appointments with the psychologists will last four to six hours.
Please have your primary care doctor fax us a referral form.
After receiving your referral, we will mail a packet for you to complete that includes:
- New patient parent questionnaire
- Teacher questionnaire: Age 4 and younger | Age 5 and older
- Child behavior checklists: one form for parents, one form for teachers
- Behavioral pediatrics feeding assessment
- Child sleep habits questionnaire
You and your child's teacher (if applicable) should complete the requested forms and return them to the Center for Child Development at least two weeks before your appointment. Clear and accurate information that arrives early leads to better evaluations.
It is important that your child gets a good night's sleep and eats breakfast the day of the appointment.
Arrive 15 minutes before your scheduled appointment to complete paperwork. If you arrive 30 minutes or more after your appointment time, your child will not be seen and you must call the appointment line to reschedule.
Bring the following items to your appointment:
- Insurance cards
- Copies of medical information
- Pen and paper for note-taking
- Stroller (if needed)
