We ask that families have their primary care doctor fax us a referral form. Links to the referral form are below.
When we receive the referral, we will mail a packet for parents to complete that includes:
- New patient parent questionnaire
- Teacher questionnaire: Age 4 and younger | Age 5 and older
- Child behavior checklist, one form for parents, one form for teachers
- Behavioral pediatrics feeding assessment
- Child sleep habits questionnaire
Your and your child's teacher (if applicable) should complete the requested forms and return them to the Center for Child Development two weeks prior to your appointment. Clear and accurate information, that arrives early, leads to better evaluations.
Preparing for your Visit
Appointments with your medical providers last for about 1 hour. Appointments with the psychologists will last 4 to 6 hours. It is important that your child gets a restful night's sleep and eats breakfast on the day of the appointment.
We also ask that in addition to the completed new patient packet of information, you include your child's IEP (individualized Education Plan) or IFSP (Individual Family Service Plan) when you mail it to the clinic two weeks before the visit.
It is important to arrive 15 minutes before your scheduled appointment time for paperwork. If a family arrives 30 minutes after the scheduled time or later, their child will not be seen, and they will need to call the clinic to reschedule.
Please bring with you the following items:
- Insurance cards
- Copies of medical information
- Pen and paper for note-taking
- Stroller (if needed)
