Credentialing for Vanderbilt Medical Group

Steps Required

  1. Request a credentialing packet by emailing Tonda Rice or calling (615) 936-3746.
  2. Once the completed credentialing packet is received by Provider Support Services, the credentialing process takes approximately 90 days. If the files are not processed within 90 days, a new application must be completed, and the process restarted.
  3. Instructions on how to complete the process and a documentation checklist are included with the packet.
  4. Information on applications older than 120 days is not considered current. Forms may need to be resigned and re-dated, and a new application completed before the credentialing process can continue.
  5. Packets with missing information and/or documents necessary for the credentialing process are considered incomplete and will not be processed.

Key Timelines are based on the provider's requested start date and include the date that packets must be received by Provider Support Services for processing and approval.

Credentialing Process:

The effective date of privileges maps to the Board of Trust approval date. This may be delayed to coincide with the practitioner's date of malpractice coverage. If an Allied Health practitioner needs to be approved by a certain date, the file must be presented to the Joint Practice Committee prior to the Board of Trust date that is on or before the start date.

Providers cannot start seeing patients until the effective date.

Recredentialing for Vanderbilt Medical Group
(approximate 90-day process, once received by Provider Support Service)

  1. Recredentialing is required every two years from original credentialing date.
  2. The physician should receive a recredentialing packet approximately 120-150 days prior to due date which includes a letter from the Provider Support Services requesting information be returned within 30 days. (See CV in month/year format)
  3. Packet information must be verified and/or completed including official documentation of required CMEs.
  4. Packet must be returned to the Provider Support Services at least 90 days prior to due date to avoid termination of privileges.
  5. Information will be reviewed for completeness and verified by the Provider Support Services.
