Credentialing Process for Community Pediatricians
 

The Medical Staff Office is responsible for the credentialing process. A series of data-collecting activities form a basis for decisions regarding appointments and reappointments to the medical staff, as well as delineation of clinical privileges for individual medical staff members.

General information

To obtain a credentialing packet, you must complete a New Physician Information Form and indicate your estimated start date. Email the completed form to tonda.rice@vanderbilt.edu, fax to (615) 322-1367, or mail to:

Notes

  • Requests should include a location to send the credentialing packet, or a name of the contact person for pick up and/or delivery.
  • The packet may be submitted while you are also in process of obtaining the required faculty appointment.
  • The packet must be filled out in its entirety and submitted to the Medical Staff Office.
  • Select the appropriate Medical Staff Category. All initial packets must be submitted as Affiliate. Refer and Follow may be selected at renewal. (see Refer and Follow and Affiliate Categories)

Steps for credentialing

Approximate 90-day process, once received by Medical Staff Office.   

  1. Fill out Credentialing Packet. Send CV with three letters of reference to start faculty appointment process (credentialing process and faculty appointment process can be in progress at the same time.
  2. Submit to Medical Staff Office (instructions included with packet).
  3. Information will be reviewed for completeness, and the submitted information will be verified by contacting the appropriate institution including all education, hospital/group affiliation, and malpractice claims history (copies obtained by applicant are not accepted).
  4. Once verified, the information will then be sent to the Credentialing Committee, which meets on the second Thursday of each month.
  5. After Credentialing Committee approval, information goes to the Medical Center Medical Board and then to the Board of Trust for approval.

Credentialed clinical physicians may:

  • Admit patients to Children's Hospital
  • Apply for online access to patient information.

Recredentialing for community pediatricians

Approximate 90-day process, once received by Provider Support Services. Review available Medical Staff Categories and select appropriate Refer and Follow or Affiliate Category.

  1. Recredentialing is required every two years from original credentialing date. Provided we have the physician’s current address, the physician should receive a recredentialing packet approximately 120 to 150 days prior to due date.
  2. Packet information must be verified and/or completed including official documentation of required CMEs.
  3. Packet must be returned to Provider Support Services at least 90 days prior to due date to avoid a late fee and possible termination of privileges.
  4. The information, once verified, will then be sent to the Credentialing Committee, which meets on the second Thursday of each month.
  5. After Credentialing Committee approval, information goes to the Medical Center Medical Board and then to the Board of Trust for approval.
