How can I help?

  • Outpatient Pharmacies
    Please provide the pharmacy with prescription insurance information.
  • If your child is admitted to the hospital
    Please bring all bottles of medicines your child is currently taking. The bottles contain important information such as the medicine name, dose, and pharmacy phone number where the medicines were filled.

What are the methods of payment?
Our pharmacies accept cash, check, Visa, MasterCard, and American Express. 

What will my copayment be?
The pharmacy does not know your copayment information. When your prescription is filled  in our pharmacy, the insurance company returns copayment information to us.

What information is needed to get my prescriptions filled?

  • Patient's full name
  • Address
  • Phone number
  • Social Security Number
  • Sex
  • Date of birth
  • Drug allergies, if any

We ask that patients or family members provide a prescription insurance card so the pharmacy can bill prescriptions. The insurance information used by the hospital or doctor's office may not provide adequate information to the pharmacy for prescription medicines.
