Arranging a Neonatal Transport

 

Call toll free (855) 322-9111 to let us know that you are calling for a neonate requiring transfer or consultation. Our communication center will take your name, phone number and the following information:

  • Infant's name
  • Gender
  • Gestational age
  • Birth date, time and weight
  • Clinical problem

You will then be transferred to the admitting neonatology fellow or attending to discuss  the infant's condition, what treatment has been performed, and a decision will be made regarding the need for transport.

If transport is necessary, the Neonatal Transport Team has the capability of transporting by ground or air and is available to transport those infants requiring acute stabilization and specialized care during transport.

When an infant is transferred via our team, the following items should accompany the infant:

  • Photocopy of mother's prenatal history
  • Photocopy of mother's and baby's medical records, including all nurses' notes
  • Completed medical necessity form faxed to you prior to transport
  • Duplicates of all the baby's x-rays
  • Discharge summary for babies with prolonged hospitalizations
  • The Vanderbilt Hospital Admitting Department will call to get information about the family's residence and insurance coverage. This, however, will not preclude or delay the dispatch of the Angel Transport Team.
