Planning Your Visit

 

Before your visit

New patients (newborns)

  • Enroll your newborn on your insurance plan.
  • Contact us to complete the intake process.
  • Arrange for insurance referral authorization for each doctor and therapist, if essential for insurance coverage

New patients (existing treatment plan)

  • Contact us to complete the intake process.
  • Fax copy of insurance card to Spina Bifida Program Office at (615) 936-5543. 
  • Arrange for insurance referral authorization for each doctor and therapist, if essential for insurance coverage.
  • Bring copies of current medical records and hard copies of most current diagnostic reports and films (CTs of head; MRI of head and spine; scoliosis, hip, and other pertinent plain films, urodynamics printouts, renal ultrasounds, VCUGs). Referring doctors or primary care physicians can help with this request. Mail information to:

Return patients
Arrange for insurance referral authorization for each doctor and therapist, if essential for insurance coverage.

On the day of your visit
The Spina Bifida Clinic process begins at 1:00 p.m.

Return patients may have previously recommended diagnostic studies scheduled before the clinic visit. We make every effort to coordinate these visits with the family’s needs in mind. Most families ask that the tests be arranged during the morning before the clinic appointments start. Please contact us if you prefer other arrangements. The time and place of every diagnostic test is included in appointment notifications you will receive.

Your appointment with the multi-specialty team will take up most of the afternoon, so plan to eat lunch before clinic starts. Your appointment with the Spina Bifida Clinic will be different from a simple doctor visit. Each appointment is an extended outpatient stay. Each family will have an opportunity to meet with one or all of the team members. Families may experience a waiting period between visits with specialists due to the natural flow of the clinic program.

What to bring

  • Medical questions for each doctor and professional
  • Copayment for each doctor and therapist as outlined by your insurance coverage
  • Insurance referrals and insurance card(s)
  • Social Security card
  • Emergency contact information
  • Braces and splints (to every clinic)
  • Wheelchair (to every clinic)
  • Diapers or continence supplies
  • A complete list of all medicines or bring the containers
  • Notes from community doctors
  • Activities or toys
